All new applicants must submit a complete file before they are considered for scholarship assistance.

A complete file consists of the following:

1. Scholarship Application
2. TRIBAL ENROLLMENT VERIFICATION
3. Copy of High School Diploma, General Education Diploma or DEGREE, Diploma for College Graduates.
4. ACADEMIC PLAN OF STUDY .
5. Registration/Tuition Bill for semester attending
6. Unofficial/Transcript for the last college/university semester attended
7. Acknowledgement Form
8. Acceptance Letter from Institution
9. W-9

Continuing Student must ensure following documents are complete in student file for the beginning of each academic semester:

1. Continuing Scholarship Application
2. Unofficial/Official Transcript for the last semester attended
3. Registration/Tuition Bill for semester attending.
4. ACADEMIC PLAN OF STUDY
5. W-9 If any changes are made to name/address.
6. Acknowledgement Form
7. Progress Report Form
