Tribal Identification Cards:
- The Tribal Identification Card identifies tribal members as enrolled members with a federally recognized tribe;
- They are issued to Pascua Yaqui Tribal Members 16 yrs. and older (minors must be accompanied by parent(s) or legal guardian(s));
- Replacement cards are $5.00 to paid in the form of a money order made payable to the Pascua Yaqui Tribe Enrollment Dept.;
- Tribal IDs are issued at the Enrollment Department on Mondays, Wednesdays and Fridays from 8:00 am to 5:00 pm (with exception to every second Friday of every month)
- Tribal Members 55 yrs of age and over are not charged a replacement fee.
Yaqui Communities Enrollment Update Visits:
To assist tribal members who do not live on the reservation, the Enrollment Department

conducts update visits to the identified Yaqui communities and provides the following services:
- Provide enrollment information packets.
- Accept completed enrollment applications.
- Issue Certificate Degree of Indian Blood (CDIB) and Tribal Identification Cards.
- Update Enrollment Information.
- To request an update visit to your YAQUI community, please complete and submit a "Community Request Form".
The Enrollment Department makes a monthly visit to the Guadalupe Modular every SECOND FRIDAY of EVERY MONTH. It is located at
9405 S. Avenida Del Yaqui, Guadalupe, AZ. 85283. Should you have any questions about these visits, please call our office at the numbers below.
