Our Mission

To promote Healthy and Safe home environments for our Elderly and Physically/Developmentally Disabled clients by providing quality home health care. Our services are designed to assist clients in meeting unique demands and challenges in their home and community.

  1. Customized Care Plans may include: Attendant Care, Personal Care and Respite Care.
  2. Provide qualified care providers to assist client on a one to one basis.
  3. Flexible schedules to meet clients’ needs.
  4. Make referrals to other tribal programs to better serve the client’s plan of care.


This Service includes routine household tasks such as:

  • Routine cleaning: light housekeeping, sweeping, mopping, cooking and dusting of clients home as well as assistance with laundry.
  • Assistance with meal planning, shopping, storage, and food preparation.
  • Run errands such as: post office, paying bills, and other errands as needed to assist client.


Referrals are made to the Community Health Representative (CHR) Program when clients are in need of personal care assistance.


Referrals are made to the Community Health Nurse (CHN) Program for assistance with preparing medication boxes and medication box delivery as needed by client.


Home Community Based Services Program (HCBS) provides support to the family caregiver by:
  • Providing respite care when caregiver attends support groups to better understand client health condition.
  • Caregivers reports daily changes in clients condition to ensure that all services that the client needs are being met.
