Higher Education Assistance Program

New Summer Session Information

All forms must be typed or in fillable format including any/all student identifiers such as name, student ID number.

  • Unofficial/Official Transcripts for any college or university attended.
  • Degree Plan, DARS, SAPR, Advisement Check Sheet, or Advisement Report (MUST be obtained from student web portals and contain student’s name and field of study/major consistent with application)
  • Schedule of Classes
  • Tuition Bill and Required Book Prices
  • Copy of your HS Diploma or GED (for all NEW and RETURNING students)
  • Students must complete the “Free Application for Federal Student Aid” (FAFSA) in accordance with the federal policies.
    As proof of submittal, a Student Aid Report (SAR) must be included in your application packet.
