The 2022 Yaqui Education Services Back-to-School Assistance Program will be open for applications beginning June 17th, 2022 thru August 12th, 2022. The program is open to all Kindergarten thru 12th grade students. Students must be registered members with the Pascua Yaqui Tribe and an Arizona resident. - PYT Education Department Release of Information (ROI) signed for each student- Click Here for Form
- Proof of 2022/2023 school enrollment for each K-12 student, within the state of Arizona.
- PYT Tribal Enrollment for each student.
- Completed W-9 form by Parent/Legal Guardian. Please be sure to attach the required W9. Not doing so may delay the processing of your request. – Click Here for Form
- Proof of Income for the past 90 days.
