Arizona Long Term Care

AHCCCS ALTCS – Arizona Long Term Care

This program provides quality long term care services to financially and medically eligible tribal members at little or no cost.

Services range from:

  • Doctor’s office visits
  • Hospitalization
  • Major surgery
  • Nursing home placement
  • Palliative Care and Hospice
  • Prescriptions
  • Routine laboratory service

Other services include:

  • Visiting nurse
  • Home delivered meals
  • Transportation
  • Attendant Care Provider – hours to be determined by medical necessity
  • Medically necessary Durable Medical Equipment
Application

The application process has two parts:

Part One — is the financial eligibility which includes resources and income (an authorized person can act on your behalf during the application process). You will need to provide documents to show that you meet financial and non-financial eligibility requirements.

Part Two — is the medical eligibility requirement.

You must meet both criteria to become enrolled in an ALTCS plan. The application process is reviewed and the final determination is made by AHCCCS.

ALTCS Eligibility Criteria
  • Must be a U.S citizen or have Legal Resident Status
  • Must have a Social Security Number
  • Must reside in Arizona
  • Must have care needs that equal what is provided in a skilled nursing home
  • Must meet financial and medical eligibility requirement
