AHCCCS ALTCS – Arizona Long Term Care
This program provides quality long term care services to financially and medically eligible tribal members at little or no cost.
Services range from:
- Doctor’s office visits
- Hospitalization
- Major surgery
- Nursing home placement
- Palliative Care and Hospice
- Prescriptions
- Routine laboratory service
Other services include:
- Visiting nurse
- Home delivered meals
- Transportation
- Attendant Care Provider – hours to be determined by medical necessity
- Medically necessary Durable Medical Equipment
ALTCS Eligibility Criteria
- Must be a U.S citizen or have Legal Resident Status
- Must have a Social Security Number
- Must reside in Arizona
- Must have care needs that equal what is provided in a skilled nursing home
- Must meet financial and medical eligibility requirement
